  • Blood pressure and pulse
  • Urinalysis
  • Physical examination of heart, lungs, abdomen, spine and neurological function
  • Review of any conditions requiring documentation, such as diabetes, sleep apnea or cardiac history

Bring with you

  • Photo identification and your driver’s license
  • Eyeglasses, contact lenses or hearing aids if you use them
  • A complete list of your medications
  • Letters from your treating physicians for any managed condition
  • Your CPAP compliance report if you are treated for sleep apnea
  • Your most recent A1c if you have diabetes

Bring the documentation

The commonest reason a driver leaves without a certificate is missing paperwork for a condition that would otherwise pass. Gather the letters before you come.

How long is the certificate valid?

Up to twenty-four months. Certain conditions require a shorter certification period, which the examiner will explain.

Do I need an appointment?

Walk-in is usually fine, but calling ahead means we can confirm a certified examiner is on site at the time you plan to come.

What does it cost?

DOT examinations are not a covered insurance benefit and are paid at the time of the visit. Call either clinic for the current price.
