There are no contact forms, intake forms, appointment forms, newsletter sign-ups or chat widgets anywhere on this website. That is deliberate. Personal health information submitted through an ordinary website form ends up in the web host’s database, logs and backups, and we will not put your information there.
Every way of reaching us from this site — phone, the patient portal, online scheduling — leads to a platform covered by a business associate agreement and designed to hold health information safely.
Technical information
Our web host records standard server logs, including the internet address your request came from, the page requested, and your browser type. We do not use advertising, tracking or analytics tools. Server logs are not used to identify you and are never combined with medical records.
Cookies
This site sets no advertising or tracking cookies. Your browser stores one preference locally — your chosen text size — which never leaves your device.

Our patient portal and online scheduling pages are operated by third parties under agreement with us. Once you follow those links you are on their systems and subject to their privacy terms.
